PDF Forms
These two samples are variations of the same form. The first is image-only, with no fields. The second is PDF Normal (edited & formatted) with fields that can be populated.
Fields can be inserted on any type of PDF form, whether it's image-only, image with text, or PDF normal.
The PDF Normal form to the right can be populated with data, and then printed. It will reset to blank fields after closing.
To experiment, try populating a few fields and then printing the form. Close the form and then reopen. You'll find the fields are blank; the form has been reset for the next user.

The sample scans on this page are based on the same document. At right are a color and grayscale flat-bed image of the document scanned at 95 dpi. Although higher resolutions are possible in some cases, due to file size limitations these examples have been kept to a minimum. Some images may require magnification to view properly. Normally, TIF (tagged image format) images are used as an industry standard. These two files have been converted to BMP format for easier viewing. |
